The same procedure as for a "New Passport" has to be followed by submitting the same application and documents, together with the following:-

Fee : $.575/=, payable by cash, bank cheque or money order to the "Consulate General of Sri Lanka" |
|
|
A Police Report from the nearest Police Station |
|
|
Photocopies of Lost Passport (if available) |
|
|
Proof of Identity (NIC, Bank Card, Driving Licence, etc.) |
Please read ALL INSTRUCTIONS contained in the printed application forms carefully and complete same without leaving any 'blanks', as incomplete applications will be rejected.
Kindly note that it would take at least 8 - 10 weeks to process an application for a new passport.
